  2. Smart Devices: You will receive easy-to-use medical devices (like a blood pressure cuff or glucose monitor) that work right out of the box—no Wi-Fi or complicated setup required.

  3. Proactive Monitoring: As you take your readings at home, the data is shared instantly with Nsight’s 24/7 clinical team and integrated into your records at Chantilly Family Medicine.

  4. Clinical Intervention: If your readings are outside of your target range, a nurse will reach out to check on you and coordinate directly with your Chantilly Family Medicine doctor if a change in treatment is needed.
