  • Please note: As of January 20, 2025, COVID-19 vaccination is no longer a requirement for immigration medical examinations.

4. Communicable Disease Screening

  • Tuberculosis (TB) Testing: An Interferon Gamma Release Assay (IGRA) blood test is typically required for applicants aged 2 years and older. Further evaluation (such as a chest X-ray or sputum tests) may be necessary if the IGRA test is positive or if you exhibit symptoms.
  • Syphilis Screening: A serologic blood test for syphilis is required for applicants aged 18 to 44 years.
  • Gonorrhea Screening: A urine test for gonorrhea is required for applicants aged 18 to 44 years.
  • Screening for other conditions, such as Hansen’s Disease (Leprosy), may also be conducted as per the latest CDC guidelines.

5. Mental Health Assessment

How long does the immigration medical exam take?

The appointment itself typically takes 45-90 minutes, depending on individual needs and the number of vaccinations required. However, plan for additional time for lab work if done off-site, or if a follow-up visit is needed for a positive TB test result.

What if I don’t have my vaccination records?

If you don’t have records, we can perform a blood test (titer) to check for immunity to certain diseases. If immunity is not present, or if titers aren’t available for a specific vaccine, you will need to receive the required vaccinations.

What if my TB test is positive?

A positive IGRA (blood) TB test does not necessarily mean you have active TB disease. It means you have been exposed to TB bacteria. Our civil surgeon will order a chest X-ray. Depending on the X-ray results and any symptoms, further evaluation (like sputum tests) or referral to a specialist for treatment of latent TB infection may be necessary.

Can I open the sealed I-693 envelope?

Absolutely not. The sealed envelope is for USCIS. Opening it will invalidate the form, and you will likely need to repeat the examination and pay for it again.
